ADDITIONAL SERVICES

In addition to providing primary health care needs, Central Virginia Health Services works to provide its patients additional services.

Health Disparities Collaborative
The Health Disparities Collaborative is a model of care focused on eliminating health disparities and enhancing the delivery of care to improve patient outcomes. The focus is diabetes; however, because of the link to cardiovascular disease, key clinical indicators for both conditions are monitored. Patients enrolled in the Collaborative have demonstrated improved clinical outcomes and are able to maintain better indicators over an extended period of time.

Reach Out and Read
Reach Out and Read (ROR) promotes early literacy and school readiness in pediatric exam rooms nationwide by giving new books to children and advice to parents about the importance of reading aloud. ROR trains doctors and nurses to advise parents about the importance of reading aloud and to give books to children at pediatric checkups from 6 months through 5 years of age, with a special focus on children growing up in poverty. By building on the unique relationship between parents and medical providers, Reach Out and Read helps families and communities encourage early literacy skills so children enter school prepared for success in reading. The Reach Out and Read program is available at Central Virginia Community Health Center, Southern Albemarle Family Practice, the Community Health Center of the Rappahannock Region, and Charlotte Primary Care.

Komen Mammography Program
All CVHS sites receive a limited amount of Komen funding.  Women who are uninsured or underinsured may be eligible to receive free screening mammograms under the Komen grant program.
